As a rule, an informal letter to the examination board is sufficient. This must include the student's data (name, course, matriculation number, address and email), the request in as much detail as possible (what is it exactly about?), a statement of reasons (why should the examination board approve the application?) and at the end a specific application (what should the examination board decide on?). In addition, you should - if available - attach relevant evidence (e.g. certificates, transcripts of grades) to the letter.
